This page contains important information about professional services and business policies offered by Amara Brook, PhD, ABPP at Flow Psychological Services, PC and Rachel Leah Kraus, LCSW-C at Kraus Psychotherapy, LLC. Please read it carefully and feel free to email Dr. Brook with any clarifying questions. When you submit this document, it will represent an agreement between you or your agency, hereafter referred to as “Client,” and Amara Brook, PhD, ABPP at Flow Psychological Services, PC and Rachel Leah Kraus, LCSW-C at Kraus Psychotherapy, LLC, hereafter referred to as “Consultants” or “Consulting Professionals.”
This agreement is made between Client and Consultants. The Client hereby contracts the Consultants to provide the services outlined and stipulated as below. 1. Services to be Provided
Synchronous group consultation sessions focused on tailoring DBT to the needs of Autistic and other neurodivergent clients.
2. Independent Contractors
The Consultants shall perform the services in a completely independent manner and under their sole responsibility. Both parties agree that the relationship between Client and Consultants shall at all times be considered that of independent contractors. The Client shall remain solely responsible for their own compliance with all federal, state, and local laws and ethical guidelines.
3. Nature of Consultation
The Client and Consultants understand that these consultations do not constitute clinical supervision and that the Client remains completely responsible – ethically and legally – for the decisions they make in their own clinical case situations. Consultants will provide Client with an opportunity to discuss clinical cases and issues about which they may have some expertise, and they may help Client consider options for responding, but the comments made for Client’s consideration are not supervisional mandates.
The Client and Consultants also understand that although they may sometimes need to discuss personal issues that may be relevant to Client’s clinical work, these consultation services do not constitute psychotherapy.
The Client and Consultants understand the potential limits of the confidentiality of this relationship. To the extent possible, Client’s case presentations will provide no identifiable patient information. However, Client understands that if they provide identifiable information about a situation regarding which Consultants have an ethical or legal obligation to report confidential information, they will inform Client at the time and will give Client the opportunity to make the report themself.
4. Consent to Record
Consultants will record the group consultation sessions in a HIPAA compliant manner and later use de-identified questions and answers in written publications and/or trainings.
5. Professional Fees and Billing
Consultants’ hourly group consultation fee is $100 per hour. Payment is due when Client enrolls for the consultation session.
6. Cancellation policy
Once you are signed up for a group consultation session, refunds will not be provided. However, if you provide us 48h notice (by emailing [email protected]) that you will be unable to attend the originally scheduled session, you can apply the fee to a future session.
By filling out the below form and checking the box, the named acknowledges that you have read the information in these terms, agree to abide by its terms, and give your consent to receive consultation services provided by Consultants. This consent is voluntary and either party may revoke consent in writing at any time.
